Job Description: Client Navigator

The cancer center and breast care client navigator's primary focus is to provide seamless integration of cancer care in a multidisciplinary setting. The navigator is a liaison for patients. families, direct care providers, and ancillary clinics, community providers, and the health care system to ensure continuity of care.

Duties include but are not limited to:

  • Under the direction of breast service providers, assist with coordination and continuity of care.
  • Serves as point of contact for patients and physicians for incoming/out going referrals.
  • Guides patients transferring on how to obtain and submit out side medical records, including pathology slides if available.
  • Orients new patients to clinic as needed, provides support and concrete services.
  • Facilitates interactions and communication with health care staff and providers.
  • Schedules patients for appointments.
  • Refers patients to financial counseling as needed.
  • Ensures patients are scheduled for appropriate follow up care and imaging.
  • Coordinates access to up-to-date cancer patient information (of all media)
  • Tracks interventions and outcomes.
  • Connects patients to hospital, community and social support services.
  • Identifies and mitigates barriers to care in collaboration with social work.
  • Links patients to hospital and community resources.
  • Advocates for patients at the facility and the broader health care system.
  • Identifies and coordinates with sources for free resources.
  • Schedules radiology and other appointments.
  • Following up on providers' orders, schedules mammograms, breast ultrasounds and other imaging appointment.
  • Facilitates linkage to MRO guided biopsy appointments at NYU Langone.
  • Facilitates appointments.
  • Coordinates screening and outreach programs.
  • Organizes breast cancer month (October) and mother's day (May) screening outreach campaigns.
  • Liaisons with providers, radiology/mammography, and public affairs to organize events.
  • Reports to Breast Program Leadership (BPL) meetings on outreach data and screening outcomes.
  • Participates in other outreach events as appropriate.
  • Conducts patient satisfaction survey by phone to gather breast patients' feedback about their care and experience.
  • Coordinates with other staff conducting surveys.
  • Records responses and comments.
Education Requirement: Associates degree

Knowledge, Skills, Abilities: Bilingual in Spanish.
